Glendale Tennis Club Web-printable Membership Application

If you were looking to register competely over the web - sorry, but we still need a signed application from you, and some form of payment, of course.

But to make things as fast as possible, just print our application form, complete it and mail it in, with your payment. We will mail you back your shoe-tags (required to play) and our newletter and calendar so you know what's going on at your club and can best enjoy your membership. If you are joining a league or team, the coordinator or captain will contact you by phone.

Completing Your Membership Application

  1. Fill in the top part of the form. This tells us about your family members who wish to play at Glendale. There's lots of activities you can join, leagues and lessons and more! Check out the items on the side-bar and the details on the registration form.

    2. Don't forget to sign the form. We cannot accept it if it is not signed. Registration for a single junior member must be signed by a parent or legal guardian.

  2. Complete the bottom of the form: This is for calculation of your membership fees and includes payment for basic single or family membership, any lessons, and leagues, and a mail-in fee.

  3. Make out a cheque for the total. (Sorry about that part.).

  4. Mail your cheque and the completed form (signed) to Glendale Tennis Club - Membership, P.O. Box 444, Pickering ON, L1V 2R6. You should expect your memership package (toe-tags, newsletter and calendar) by return mail within 10 days.
